Returning Student Form

Once again Susquenita School District will be using Infosnap to collect returning students' update health and demographic information.

It is important that the schools have the most current information on your child before the start of the school year. With this in mind, please complete the form no later than July 30th for any student participating in a Fall sport and August 6th for all others.

You will either be receiving an email or a letter in the mail this week or next outlining the steps you need to take in order to complete the form. If you do not receive anything by the end of next week, please email Pat Woods at pwoods@susq.k12.pa.us.
