The fundraiser approval request form can be found HERE.  It is formatted as a fillable PDF, so you may type your answers on form and print it out.

Please submit it to the building principal no less than ONE WEEK prior to the start of your fundraiser.

Questions may be directed to the building principal where the fundraiser will be held.
